Introduction

The malaria vaccine (MV) was introduced in Cameroon in 2024. In its first phase, 42 districts across the 10 health regions were enrolled. Three districts in the Littoral Region are implementing it. This study aims to analyse its early impact on severe malaria cases in children under 5 years of age.

Methods

A descriptive and comparative analysis of annual trends (2020-2024) of cases recorded in DHIS 2 was conducted. Variations in 2024 were compared with pre-vaccination trends (2020-2023).

Results

In 2024, a11.2% increase in severe malaria cases was observed compared to 2023, reversing the downward trend seen since 2021. Districts with the lowest vaccination coverage (MV3 at 28.75% and 32.03%) are those where the incidence of severe cases increased most. A significant negative correlation (r = -0.86) was observed between the 3rd dose coverage rate and the variation in severe cases in 2024.

Conclusion

Although preliminary, these data suggest that complete vaccination coverage is crucial for controlling severe malaria. The overall increase in cases in 2024 underscores the importance of confounding factors and the need to strengthen the completion of the vaccination schedule.
